If you have purchased ShipMonk Package Protection with your order, please enter your information below. We may request additional information before we process your claim.

Your claim must be filed within 60 days of the shipment date.

Please place your return request within 30 days of delivery of your package. If you have not received it place your request within 30 days of the latest estimated delivery date.
